clubhouse fob/card and locker application form

​FOB CARD APPLICATION:   There will be one (1) fob card available per membership.  The charge for the fob card is $ 50.00 per year. Fob card access extends from May 1 to Oct 31. To apply for a fob card please print this page, fill in the information below and MAIL to the address at the bottom of the form (ONLINE APPLICATIONS ARE NOT ACCEPTED AT THIS TIME) You must be 25 yrs or older.

Fob/cards are available on a limited basis and subject to MYC approval.
 
I am interested in renting a locker________$100.00 per season. (May-Oct).
Lockers are available on a first come basis.  The locker size is 8”w x 21”h x 12”d
(Fob/card access and locker rentals are not available to Junior members)
